Acute lacunar cerebral infarcts

Presentation

Right upper and lower limb numbness and tingling sensation, slurred speech for ~24 hours.

Patient Data

Age: 65 years
Gender: Male

Ill-defined hypodensities in the left thalamus and corona radiata in line with small ischemic infarcts. 

One day later

Rapid stroke protocol MRI: 

Both the left thalamic and corona radiata infarcts are hyperintense on FLAIR, but only the thalamic lesion restricts on DWI confirming it as an acute infarct. There is no DWI-FLAIR mismatch. 

Case Discussion

The CT appearance of subacute and acute lacunar infarcts can be identical, whilst MRI can readily differentiate on the basis of restricted diffusion, which gradually disappers during the subacute stage of ischemia.
