Presentation

Worsening headache and neurological deficits.

Patient Data

Age: 35 years
Gender: Male

Large left convexity extra-axial fluid collection of variable high/low density. Rightward midline shift and subfalcine herniation with trapping of the right lateral ventricle. Likely early uncal herniation. 

Case Discussion

Typical findings of acute on chronic subdural hematoma with severe mass effect and herniation. This young patient had a history of alcohol abuse, likely resulting in repetitive head injuries causing the bleeding.
