Case contributed by Bahman Rasuli
Diagnosis almost certain

Acute severe abdominal pain in a patient with COVID-19 RT-PCR positive test and pneumonia.

Patient Data

Age: 15 years
Gender: Male
  • diffuse enlargement with irregular pancreatic outline accompanied by peripancreatic fat stranding and spread of an inflammatory exudate to the lesser sac, left paracolic gutter and retroperitoneal space
  • the adjacent fourth segment of the duodenum and proximal jejunal loop demonstrates mural thickening, in keeping with reactive changes
  • no signs of necrosis
  • left-sided pleural effusion

Case Discussion

Findings are compatible with acute pancreatitis and confirmed biochemically with markedly elevated amylase and lipase.

The recent case was a healthy person with no past history of a specific disease, laboratory, or metabolic abnormalities and just received five doses of remdesivir for five days before the admission because of COVID-19 pulmonary infiltrations.

Currently, the association between COVID-19 and acute pancreatitis has not been firmly demonstrated in the literature 1.
