Acute pancreatitis - Balthazar E

Case contributed by David Cuete
Diagnosis certain

Presentation

Acute abdominal pain.

Patient Data

Gender: Male

CT abdomen and pelvis

ct

CT shows diffuse increased volume of the pancreas, with peripancreatic inflammatory changes associated with increased adipose tissue density and laminar liquid in the mesenteric root and both anterior pararenal spaces, where it reaches a maximum thickness of approximately 15 mm to 23 mm from right and left. Further noted is a slight amount of free fluid in the left sub-phrenic space. No alterations were observed in pancreatic impregnation pattern. No dilation of intra or extrahepatic bile duct. Increase enhancement is observed of the bile duct wall, finding probably determined by contiguity. In the medial pole of the left adrenal gland is seen a nodule of 20 mm and an 11 mm in the right adrenal gland, with the characteristics of adenomas.

Case Discussion

Findings consistent with acute pancreatitis, Balthazar Grade E, without tomographic signs of glandular necrosis.
