Left-sided facial and neck tender swelling.
Patient Data
The left parotid gland shows marked diffuse enlargement with indistinct margins and heterogeneous attenuation associated with edema of the adjacent subcutaneous soft tissues. No collections or abscesses in the parotid space. No glandular or ductal stones.
Multiple enlarged left intraparotid and upper deep cervical lymph nodes. The left submandibular gland showed irregular borders by the enlarged nearby lymph nodes.
Opacification of left mastoid air cells and middle ear cavity with no ossicular erosions in keeping with otomastoiditis. Diffusely thickened soft tissue lining of the left external auditory canal, reflecting associated otitis externa.
Case Discussion
This case demonstrates parotid sialadenitis secondary to otomastoiditis. Otomastoiditis can spread infection to nearby anatomical structures. Parotid sialadenitis can be due to obstruction of Stensen's duct with a stone (sialolith) or infection.
