Acute pontine infarct

Presentation

Dizziness and disequilibrium.

Patient Data

Age: 50 years
Gender: Female
MRI

Large high T2, low T1, avid water restriction abnormality in the right hemipons.

High T2/FLAIR signal foci in the subcortical and periventricular white matter of both cerebral hemispheres, consistent with chronic small vessel ischemia.

No hemorrhagic component.

Normal basilar artery.

Case Discussion

Regarding the above imaging findings, an acute pontine infarct is the main diagnosis.
