Acute right MCA infarct on CT perfusion

Presentation

65y old female presented with acute onset left sided weakness and neglect.

Patient Data

Age: 65 yrs
Gender: Female
Modality: CT

Plain CT demonstrates no hemorrhage and hence CT perfusion was done.

CT Perfusion shows evolving large right MCA distribution ischemia with almost all core and very minimal penumbra.CBF and CBV of 2.6 and 0.2 respectively to suggest infarct core.

Modality: CT

CT Angiography confirms a proximal M1and AI segment of MCA and ACA thrombus in right MCA.There was no dense MCA sign on plain CT.

Case Discussion
