Acute sacroiliitis - bilateral

Abnormal subchondral bone marrow signal involves both surfaces of the bilateral sacroiliac joints and displays low signal on T1 and high signal on T2WI and fat-suppressed images due to the acute inflammatory process (osteitis).
Preserved joint space of ligaments and synovial parts of the right sacroiliac joint.
No subchondral sclerosis or cystic changes. Normal shape and signal intensity of the muscle attachments. No enthesitis.
Case Discussion
MRI findings are consistent with bilateral acute sacroiliitis.
