Extensive bone marrow oedema involves both surfaces of the left sacroiliac joint and displays low signal on T1 and high signal on T2 fat-suppressed images due to the acute inflammatory process (osteitis).

Joint effusion with oedematous infiltration of the ipsilateral iliacus, piriformis, gluteus medius and minimus muscles.

Normal appearance of the right sacroiliac joint.

MRI findings are consistent with left-sided acute sacroiliitis.
