Acute venous sinus thrombosis

Case contributed by Dr Karim Hussein

Presentation

2 days of worsening headache with associated nausea and vomiting. Left sided headache made worse on bending forward.

Patient Data

Age: 20 years old
Gender: Female
Modality: CT

Plain Brain CT shows an area of hyperdensity corresponding to the Left transverse sinus and sigmoid sinus.

CT venogram demonstrates a filling defect extending from left transverse sinus and sigmoid sinus.

Case Discussion

On plain CT imaging venous sinus thrombosis appears hyperdense.

This however can be difficult to distinguish from a normal venous sinus which normally appears slightly hyperdense compared to brain tissues.

In such cases, CT venogram can help confirm the diagnosis by demonstrating filling defects.
