Acute Weber A distal fibular fracture

Combined inversion and plantar flexion superimposed on inversion trauma weeks earlier. Scan performed 2 hours after trauma.

Patient Data

Age: 30 years
Gender: Male

Avulsion fracture of the distal fibula. Maintained alignment of the ankle mortise.

Thickened intermediate signal ATFL, likely ruptured in previous trauma. Calcaneofibular ligament demonstrates high signal, but appears continuous. High signal in deltoid ligament and focal bone marrow edema in medial talus reflect contusion from inversion. Talotibial joint effusion.

Other ligaments, articular cartilage surfaces and tendons preserved.
