Adenoid cystic carcinoma of salivary gland

Diagnosis almost certain

Presentation

Painless swelling within the right floor of the mouth for 6 months.

Patient Data

Age: 50 years
Gender: Female

There is a subtle well-defined oval-shaped enhancing lesion in the right sublingual space indenting the right genioglossus muscle, obliterating the right sublingual fat and abutting the mandible without bony destruction.

Incidental left thyroid nodule.

No cervical adenopathy.

There is a lobulated hypoechoic solid lesion with internal vascularity within the right sublingual space.

The left sublingual, both parotid and submandibular glands are unremarkable.

Case Discussion

The histopathological report was consistent with adenoid cystic carcinoma.

These images illustrate how head and neck masses can be subtle on CT scans. Adding another imaging modality like ultrasound/MRI can help in detection.
