Presentation
Trauma to the right face with increased swelling
Patient Data
Complete opacification of the right maxillary sinus with poor definition of its walls.
Large tumor in the right maxillary sinus destructing the sinus walls, the zygomatic bone, the adjacent hard palate and the maxillary alveolus. Anteriorly, the mass extends to the premaxillary soft tissue. Posteriorly, there is an extension to the retroantral fat, pterygomaxillary fissure and the pterygopalatine fossa. Superiorly, the mass extends to the floor of the right orbit. The mass measures 5.5x4.5x4.5 cm in its maximum dimensions.
No cervical lymphadenopathy.
There is evidence of right radical maxillectomy and orbital exenteration.
There is a large muscular flap occupying the orbitomaxillary defect which shows a normal signal intensity and enhancement with no evidence of tumor residue.
Case Discussion
Pathologically proven adenoid cystic carcinoma of the right maxilla (T4b,N0).
