Adenoma malignum of the cervix

Case contributed by Jan Frank Gerstenmaier

This patient presented with profuse watery vaginal discharge.

Patient Data

Age: 40 years
Gender: Female

MRI pelvis

There are multicystic lesions extending from the endocervical glands to the deep cervical stroma with solid components, forming an annular mass.

The patient underwent hysterectomy. Histopathology confirms adenoma malignum

Case Discussion

Adenoma malignum can appear deceptively benign at histology and imaging alike, but the prognosis is generally unfavorable as it disseminates to the peritoneal cavity early, and response to chemo- and radiotherapy is poor.

Unfortunately no contrast was given in this case, but enhancement of the solid components would have been expected.
