Incidental finding in a 30 year old man, who complained of upper abdominal pain and in whom pancreatic enzymes were repeatedly elevated.

Thick-slice projectional MRCP showing multple fluid-filled, enlarged Rokitansky-Aschhoff sinus in the fundus of the gallbladder.
Typical finding of fundal adenomyomatosis of the gallbladder, with intramural enlargement of the Rokitansky-Aschhoff sinus, similar to diverticula. This has been termed the pearl necklace sign.
