Incidental finding, in context of upper abdominal pain with repeatedly elevated pancreatic enzymes

Patient Data

Age: 30 years
Gender: Male

Thick-slice projectional MRCP showing multiple fluid-filled, enlarged Rokitansky-Aschhoff sinuses in the fundus of the gallbladder.

Case Discussion

Typical finding of fundal adenomyomatosis of the gallbladder, with intramural enlargement of the Rokitansky-Aschhoff sinuses, similar to diverticula. This has been termed the pearl necklace sign.
