Adhesive capsulitis - shoulder

Diagnosis almost certain

Presentation

Pain and restriction of movement in the right shoulder.

Patient Data

Age: 50 years
Gender: Male

Thickening of inferior glenohumeral ligament, measuring approx. 6.5 mm in maximum thickness with edema in axillary recess. Mild thickening and edema of coracohumeral ligament are also seen. 

Type III acromion is seen with acromioclavicular joint arthropathy, eburnation along articular aspect and subtle subchondral marrow edema.

Significant supraspinatus tendinosis is seen with no obvious tear. 

Mild subscapularis tendinosis is also seen with no obvious tear.

Infraspinatus and biceps tendons are unremarkable 

Degeneration of superior glenoid labrum is seen. 

Glenohumeral articular aspects are unremarkable. 

Major myofascial structures around the shoulder joint appear normal.

Case Discussion

MRI findings are consistent with adhesive capsulitis, especially in the clinical setting of pain and reduced movement of the shoulder.
