Adhesive small bowel obstruction

Case contributed by Chris O'Donnell
Diagnosis certain

Presentation

Abdominal pain, nause and vomitting. Previous history of colonic resection for carcinoma.

Patient Data

Age: 65
Gender: Female

Dilated fluid-filled small bowel loops with abrupt transition to collpsed small bowel associated with a focal kink and narrowing of the lumen.

Case Discussion

Detection of a transition point is important for bowel obstruction interpretation i.e. the site of obstruction and possible cause. Adhesions are the presumed mechanism in many cases (following previous surgery) especially small bowel obstruction but rarely is the exact point of adhesive obstruction seen on CT as a focal kink and narrowing of the lumen by extrinsic compression.
