Abdominopelvic palpable mass on physical exam and complex mass on US exam.
Patient Data
huge multilocular cystic lesion along with right adnexa
cystic portion with fluid equivalent and T1/T2 high signal content (hemorrhagic fluid or thick proteinaceous component)
solid tissue at the inferior part of the lesion shows marked water restriction on DWI mages and enhancement equal to the myometrium
smaller multilocular cystic lesion with the same imaging features along with left adnexa
no evidence of peritoneal, mesenteric or omental nodularity or irregular thickening
minimal fluid signal at cul - de - sac space
no ascites
Case Discussion
MRI features are in keeping with a huge multilocular cystic right adnexal lesion with enhancing solid tissue and a similar smaller mass on the left side compatible with O-RADS MRI score 4 (PPV ~50%).
