Adnexal multilocular masses (O-RADS MRI 4)

Presentation

Abdominopelvic palpable mass on physical exam and complex mass on US exam.

Patient Data

Age: 45 years
Gender: Female
  • huge multilocular cystic lesion along with right adnexa

  • cystic portion with fluid equivalent and T1/T2 high signal content (hemorrhagic fluid or thick proteinaceous component)

  • solid tissue at the inferior part of the lesion shows marked water restriction on DWI mages and enhancement equal to the myometrium

  • smaller multilocular cystic lesion with the same imaging features along with left adnexa

  • no evidence of peritoneal, mesenteric or omental nodularity or irregular thickening

  • minimal fluid signal at cul - de - sac space

  • no ascites

Case Discussion

MRI features are in keeping with a huge multilocular cystic right adnexal lesion with enhancing solid tissue and a similar smaller mass on the left side compatible with O-RADS MRI score 4 (PPV ~50%).
