Diagnosis almost certain

Presentation

Incidental finding.

Patient Data

Age: 55 years

The signal drop in the out-of-phase imaging within the right adrenal lesion is due to intracellular fat.

Right-sided para midline abdominal wall defect containing omental fat in epigastric region is seen.

The gall bladder is not seen.

Case Discussion

Chemical shift MRI imaging has high sensitivity and specificity for identifying adrenal adenoma.  Signal drop in the out-of-phase sequences is related to the microscopic lipid-rich content of adrenal adenomas.
