Adrenal extramedullary hematopoiesis

Presentation

A case of beta thalassemia underwent splenectomy six years ago. Now presented with right hypochondria pain. US revealed a right suprarenal mass.

Patient Data

Age: 15 years
Gender: Male

Evidence of large soft tissue mass with course calcification is seen at the right adrenal region. It measures 7.5 x 8 x 10 cm AP X Tr and CC respectively. the mass is seen displacing the right liver lobe anteriorly as well as downward displacement of the right kidney.

Few well circumscribed posterior mediastinal paraspinal homogenous soft tissue masses, the largest measuring about 1.7 x 1.8 cm at the right paraspinal region at T8-9 disc level.

Mild perisacral soft tissue thickening.

Diffuse osteopenia of the scanned vertebrae with medullary expansion.

Hepatomegaly & history of splenectomy.

Complete surgical resection of the mass was histopathological examination revealed: tissue congested with blood and containing all hematopoietic series.

Diagnosis: Extramedullary hematopoiesis.

Case Discussion

Extramedullary hematopoiesis is seen in chronic hemolytic anemia patients including Beta-thalassemia.

It is commonly presented with hepatosplenomegaly and or posterior mediastinal masses and rarely involves other parts of the body including; the central nervous system, breast, skin, lungs, gastrointestinal tract, adrenal glands, and prostate 1.

Despite the rare involvement of adrenal glands, it should be considered as differential diagnosis in cases of chronic anemia with adrenal masses.
