Follow-up.

Patient Data

Age: 75 years

CT

Left adrenal mass measures -2 to +1 HU in density overall with focal areas of macroscopic fat present. There is some peripheral coarse calcification.

Normal right adrenal gland. Large right upper pole simple renal cysts. Cholecystectomy clips. Extrahepatic duct dilatation. Partially evaluated fat stranding and prominent lymph nodes of the small bowel mesentery is suggestive of mesenteric panniculitis.

Case Discussion

Fat density (<10 HU) adrenal lesions are most commonly adenomas, however, this lesion has atypical features such as heterogeneous appearance and coarse calcification.
