Adrenal myelolipoma - huge

There is a 16 cm mass of predominantly fat density, with mass effect on the liver. The mass is very well defined and doesn't infiltrate adjacent organs. A thin calcification is seen centrally. The most probable diagnosis is adrenal myelolipoma. The features are not suggestive of retroperitoneal liposarcoma, but it must be included in the differential diagnosis.
A hyperdense gallstone is incidentally noted.
Case Discussion
The histopathological diagnosis of adrenal myelolipoma was confirmed postoperatively. The mass was removed since the patient had chronic right upper quadrant pain, and the mass was seen as potentially contributing. Myelolipoma is usually a lot smaller (<5 cm).
