Adrenal pheochromocytoma

Presentation

Uncontrolled hypertension.

Patient Data

Age: 85 years
Gender: Male
ct

Well-circumscribed ovoid right suprarenal soft tissue mass centered on the adrenal gland, slightly hypodense to the liver on non-enhanced CT with no calcification, hemorrhage or fatty component. The postcontrast CT demonstrates a heterogeneous enhancement mainly peripheral with central necrotic areas.

Splenic mass stable on follow-up ultrasound over 3 years following surgery (probably hemangioma).

Case Discussion

CT features of a heterogeneously enhancing adrenal mass in a patient with uncontrolled hypertension, highly suggestive of a pheochromocytoma.

The patient went on to have a complete surgical resection of the right adrenal gland with a histopathological exam/immunohistochemistry study that confirmed the diagnosis of pheochromocytoma.

The MRI of the splenic mass (not available) was suggestive of hemangioma and the mass was stable on follow-up ultrasound over 3 years following surgery.
