Adult intracranial hemorrhage on ultrasound

Presentation

Presented with 2 hour history of progressively severe headache with declining consciousness level. On exam, stuporous, right pupil dilated, not moving left upper and lower extremities.

Patient Data

Age: 30 years
Gender: Male

Ultrasound through the left temporal window.

Sonosite M Turbo, P21 probe, "Pen" (Penetration mode = Low Frequency), Abdominal Preset.

Shows echogenic lesion in the right hemicortex.

Right cerebral lobar hemorrhage with 1 cm midline shift.

Case Discussion

Ultrasound may be useful to identify intracranial hemorrhage (parenchymal), even in adults. This may have applications in resource limited settings and needs to be studied further.
