Long standing poorly controlled diabetic patient with ankle deformity and swelling.

Severe degeneration and destruction with dislocation and loss of alignment are seen in the tibiotalar and talofibular joints, causing deformity and accompanied by periarticular soft tissue swelling and edema as well as superadded osteomyelitis mainly affecting distal fibular segment with lateral ankle and distal leg aspect para-osseous collection/ abscess formation.
Less involvement of subtalar and midtarsal joints as well as involvement of the 1st Tarso-meta-tarsal articulation is evident.
Case Discussion
the findings demonstrate a severe Charcot arthropathy in a old male patient with long-standing poorly controlled diabetes mellitus.
Deformity and loss of normal alignment are the first that strikes one's mind. Complete destruction of the ankle joint and the surrounding joints such as the subtalar joint and midtarsal joints may be evident. We should also look for evidence of osteomyelitis (infection of the bone). This is very important, as this needs to be amputated to prevent spread of infection.
