The MRI sequences demonstrate an intramuscular ovoid soft tissue mass centered on the left adductor brevis muscle, measuring 6.5 x 6 x 4.5 cm. It elicits a slight high signal on T1, inhomogeneous high signal on T2 with areas of restricted diffusion and heterogeneous enhancement on postcontrast sequences. There is an extension to the adductor longus muscle which is displaced anteriorly. The obturator nerve is compressed against the adductor minimus muscle. The femoral vessels are patent.

MRI features of an intramuscular mass, pathologically proven as alveolar sarcoma.
Alveolar soft part sarcoma (ASPS) is considered a rare, highly vascular, deep soft tissue mesenchymal malignancy. Usually seen in the lower extremities of young adults and account for <1% of all soft tissue sarcomas.
