Presentation

Bilateral upper and lower limbs weakness, weakness of cervical muscles, wasting of muscles, difficulty in swallowing.

Patient Data

Age: 45 years
Gender: Male

Hyperintensity is seen on T2-weighted and FLAIR images along the corticospinal tracts, most prominently in the posterior limb of the internal capsule (PLIC) and ventral brainstem. Mild diffusion restriction is seen.

Case Discussion

Amyotrophic lateral sclerosis (ALS) is the most common form of motor neuron disease, characterized by progressive degenerative changes of the upper and lower motor neurons (the condition is also called motor neuron disease) 1.

MRI findings include bilateral T2/FLAIR hyperintensity along the corticospinal tract extending from deep white matter into the centrum semiovale, internal capsule (mainly the posterior limb) and the anterior part of the brainstem. Corresponding diffusion restriction can be seen.
