Presentation

Known case of Sjogren's disease presented with severe elbow pain.

Patient Data

Age: 40
Gender: Female

Intermediate increased signal intensity in the fluid sensitive sequences is seen in the common extensor tendon representing tendinosis. Incidental note of anconeus epitrochlearis accessory muscle in the cubital tunnel associated with mild increase in the size and signal intensity of the ulnar nerve suggestive of neuritis.

Case Discussion

One potential cause of Cubital tunnel syndrome is the presence of accessory muscle in the cubital tunnel. Anconeus epitrochlearis muscle is an anomalous muscle located superficial to the ulnar nerve and may result in compression neuropathy.  In this case, the size change and signal alteration within the ulnar nerve are suspicious for ulnar neuropathy and should be correlated with the patient's symptoms.
