Aneurysm of the splenomesenteric portal venous confluence

Presentation

Epigastric pain. Incidentally noted epigastric vascular lesion on ultrasound.

Patient Data

Age: 50 years
Gender: Male

Focal dilatation of the junction of the main portal vein and the confluence of the splenic and superior mesenteric veins, reflecting venous aneurysm of the splenomesenteric portal venous confluence.

Subdiaphragmatic location of the left kidney, suggestive of subdiaphragmatic ectopic kidney.

Appendicolith is noted.

Mildly enlarged prostate.

Complete sacralized L5 vertebra (Castellvi IIIb) with L4-5 degenerative disc bulge with Vacuum phenomenon.

Case Discussion

Portal venous system aneurysm is rare 1, representing 3% of all venous aneurysms in the body 2. It is located mainly at the level of confluence, main trunk, branches, and bifurcation. It may be congenital, due to failure of closure of the right vitelline vein, or secondary to portal hypertension 3.
