Presentation

Right knee pain and a lytic lesion in distal femur

Patient Data

Age: 25 years old
Gender: Male
mri

Well defined, eccentric, expansile, multi septated lesion in the distal right femur involving metaphysis.

Fluid fluid levels can be appreciated within the lesion.

It is isointense on T1WI and heterogeneously high signal on T2WI weighted images.

Post contrast enhancement of the septations.

No extension to growth plate. No joint effusion.

No soft tissue edematous changes. 

Case Discussion

Regarding the findings, the first imaging differential is of a benign lesion, like aneurysmal bone cyst.
