Aneurysmal bone cyst of the talus

Patient Data

Age: 27 years

The talus shows a well defined lesion with thin septa and fluid-fluid level, narrow zone of transition, minimal reactive marrow edema is seen around the lesion. No cortical destruction, no pathological fracture seen.

Other findings in this case is partial tear in the anterior talofibular ligament, peroneals and tibialis posterior tenosynovitis.

Case Discussion

Major differential diagnosis will be telangiectatic osteosarcoma (TO). One should watch for any hint of aggressiveness such as periosteal reaction, cortical breakthrough and soft tissue mass to suspect TO. However, it can be entirely lytic and mimic ABC.
