Presentation

7 months of low back pain and waddling gait

Patient Data

Age: 12 years
Gender: Male

Nuclear medicine

Delayed scan shows minimal isotope accumulation in the left sacrum. 

SPECT coronal slices show photopaenic lesion in left sacrum with foci of increased isotope accumulation. 

x-ray

Expansile lytic lesion in left body of the sacrum. 

mri

Typical features of ABC, i.e. expansile mass with large cavities containing fluid/fluid levels indicating blood product. Minimal peripheral enhancement of the cystic spaces.

Note the importance of performing pre contrast T1 fat sat as fluid has relatively high signal intensity.
