A partially circumscribed bone marrow lesion with an abnormal signal is noted involving the medial meta-diaphysis of the distal right femur. There is evidence of areas of cortical break on anteromedial aspects with an exophytic extra-osseous component.

The lesion showing multiloculated cystic spaces having fluid/fluid levels.

Case Discussion

MRI features are most likely suggestive of an aneurysmal bone cyst involving the distal femur.

An important differential diagnosis would be telangiectatic osteosarcoma, that's why the patient should be followed up in an orthopaedic tumour centre.
