Case contributed by Dr Shervin Sharifkashani

Right side ankle pain and swelling following ankle torsion.

Patient Data

Age: 55 years
Gender: Male

Obliquely oriented split fracture through the width of the base of the medial malleolar process with intraarticular extension is seen. There is also an obliquely oriented fracture line with minimal displacement in the distal fibula posterolateral cortex.

On these performed X-ray series, the fracture of the base of the tibial medial malleolar process and minimally displaced obliquely oriented fracture line in distal fibula posterolateral cortex are seen.

Case Discussion

This case illustrates the ankle fracture which can be classified based on weber classification and AO/OTA classification of distal tibial fractures according to predominancy of lateral malleolar or medial malleolar fractures, respectively. The classification can clarify the mechanism of injury which determines the type of surgical plan.
