Age: 65 Years
Gender: Female

Ankle Radiograph

x-ray

Transverse fracture of distal fibula / lateral malleolus at infrasyndesmotic location.

Soft tissue swelling at lateral malleolus.

Medial malleolus appears normal.

Ankle mortise is intact.

Suggestive of Weber A fracture.

Case Discussion

Weber A fracture is infrasyndesmotic fracture of distal fibula.

It is a transverse fracture due to the pull of the adduction force applied to a supinated foot.

This is an avulsion fracture due to stretch on the ligaments on the lateral side of ankle.

Almost always this avulsion fracture is seen as a horizontal line.

This is stage 1 of Weber A and it is a pull off type of fracture resulting in horizontal fracture of lateral malleolus. Stage 1 is the most common type of Weber A fracture.

Weber A stage 2 has in addition, an oblique or vertical fracture of medial malleolus seen with push off type injury resulting from the same mechanism of adduction on a supinated foot. Stage 2 is extremely rare.

So this is a Weber A stage 1 fracture.
