Radiographs shows a spiral fracture of the lateral malleolus and a posterior malleolus fracture, both without evident cortical displacement.

Case Discussion

Despite involving the lateral and posterior malleolus, this fracture should not be confused with the bimalleolar fracture, which is an unstable fracture and almost always require surgical treatment.

Generally the term bimalleolar fracture is used when involving both the medial and lateral malleolus.
