Ankle syndesmotic injury (weight-bearing x-ray)

Presentation

Ankle injury 8 weeks ago with ongoing pain.

Patient Data

Age: 80 years
Gender: Female

Anterior inferior tibiofibular ligament is heterogeneous and thickened with loss of the normal fibrillary pattern; minimal hyperemia. Anterior talofibular and calcaneofibular ligaments have a fibrillary pattern and no hyperemia.

Asymmetric widening of the left tibial syndesmosis with a step in alignment between the lateral tibia and lateral talus. No significant left syndesmotic widening between weight-bearing and de-loading.

Case Discussion

This case demonstrates a subacute syndesmotic injury with a partial/healing tear of the anterior inferior tibiofibular ligament on ultrasound. X-rays confirm mild asymmetric syndesmotic widening but no dynamic instability/widening to indicate a "complete" syndesmotic injury involving the interosseous membrane and/or posterior inferior tibiofibular ligament.
