Anterior calcaneal process fracture

Presentation

Painful right ankle and foot following a twisting injury.

Patient Data

Age: 30 years
Gender: Male
MRI

Right ankle

Non-displaced fracture of the anterior calcaneal process with extension to the calcaneocuboid joint is present. 

Bone marrow edema /bruise also is seen at the talus and cuboid bones.   

Case Discussion

Anterior calcaneal process fractures are the result of inversion and plantar flexion injury. It represents an avulsion fracture of the bifurcate ligament. The fracture is classified as an extra-articular calcaneal fracture (does not involve subtalar joint) but can extend to the calcaneocuboid joint.
