Presentation

Patient with subarachnoid hemorrhage. No past medical history.

Patient Data

Age: 40 years
Gender: Male

CTA demonstrates anterior cerebral artery (ACA)-anterior communicating artery (Acom) fenestration complex with a saccular aneurysm. The A1 segment of the left anterior cerebral artery (ACA) is hypoplastic. 

Case Discussion

Fenestrations occur more often in the anterior circulation and the most common location is the anterior communicating artery (Acom). The association of saccular aneurysms with fenestration are very uncommon.
