Anterior cruciate ligament ganglion cyst

Presentation

Pain and limited knee mobility

Patient Data

Age: 20-25 years
Gender: Female

A well-defined oval-shaped lobulated cyst is seen occupying most of the femoral insertion of the anterior cruciate ligament (ACL) and displays a bright signal on T2 and PD fat sat images, measures 17 x 22 mm. The femoral portion of the ligament cannot be traced properly with clearly seen tibial insertion.

Case Discussion

ACL ganglion cysts involving most of the femoral attachment of the ACL. Note the delineated lobular margins of the cysts with high T2 signal, lateral displacement of ACL fibers and impingement of the posterior cruciate ligament. The clinical manifestations depend on their size, mass effect, and relationship to adjacent structures, typically diminished range of movement. They are not associated with ligament instability.
