Age: 20 years
Gender: Male

Thickening, irregularity, and signal increase of the anterior tibiofibular ligament best seen on axial T2, PDFS, and STIR with surrounding anterolateral soft tissue edema and localized fluid signal

The posterior tibiofibular and anterior talofibular ligaments appear intact.

Mild ankle effusion with distension of the posterior joint recess

Here is a case of distal tibiofibular syndesmosis injury involving the anterior inferior tibiofibular ligament (AITFL) that is a relatively frequent ankle injury
