Anterior patella apophysis

On the lateral film, there is a small focal ossific density projected anterior to the patella. This represents non-union of the anterior patella apophysis (normal variant) rather than an avulsion injury.
Case Discussion
Non-union of an anterior patella apophysis is a normal variant and does not represent an avulsion injury. Differential diagnosis: sleve fracture.
