Presentation
Mechanical fall, clinically anterior shoulder dislocation.
Patient Data
The humeral head is dislocated in the anterior inferior direction, there is evidence of a previous rotator cuff repair with surgical anchors noted within the humeral head.
There are signs of degenerative changes, however a small bony fragment just lateral to the glenoid could represent an acute Bankart lesion.
The humeral head has an irregular posterosuperior outline, this could be an associated Hill-Sachs deformity.
Case Discussion
The at-risk age groups of anterior dislocations follow a bimodal model, with both young adult men and older patients of either gender at a statistically higher risk. Anterior shoulder dislocations occur during external rotation of the abducted arm, forcing the humeral head out of the socket anteriorly, inferiorly, and medially 1.
