Left ankle inversion injury one week before the scan with lateral ankle pain and swelling after an injury and subsequent immobilization.

The anterior recess of the ankle joint shows mild effusion. There is an avulsion of the fibular end of the anterior talofibular ligament along with a bone flake. There is a corresponding donor site defect in the fibula. The ligament is intact with mild edema. The calcaneofibular ligament and anterior inferior tibiofibular ligament are normal.
