Small effusion in the anterior ankle joint recess. Bony avulsion of the anterior talofibular ligament from the fibula. Mild oedema of the anterior talofibular ligament. Normal calcaneofibular ligament and anterior inferior tibiofibular ligament.

Case Discussion
A girl developed lateral ankle pain and swelling after a twisting injury to the left ankle. An initial radiograph was negative for fracture. The ankle was immobilised for one week, and the patient was referred for an ultrasound evaluation of the ankle. The ultrasound shows fibular bony avulsion of the anterior talofibular ligament with small effusion in the anterior ankle joint recess.
