Anterior talofibular ligament injury

Presentation

A few hours prior to the presentation, a patient had right ankle inversion injury. Complaining of difficulty in walking with right ankle lateral aspect pain, swelling.

Patient Data

Age: 20 years
Gender: Female

There is a mild widening of lateral clear space of the ankle joint. Mild soft tissue swelling is present on the lateral aspect of the ankle. There is no fracture or bone lesion.

ultrasound

Anterior talofibular ligament   
- Complete tear near the fibular attachment
- Rest of the ligament shows normal echopattern

Calcaneofibular ligament    
- Intact, normal echopattern

Anterior inferior tibiofibular ligament    
- Intact, normal echopattern

Peroneal tendons         
- Intact with normal echopattern.
- No tenosynovitis.

Ankle joint - anterior recess
- effusion present

Another finding
- lateral ankle subcutaneous edema

Case Discussion

A young female presented an inability to bear weight after right ankle inversion injury. Ultrasound shows a complete tear of the talofibular ligament at the fibular attachment.
