Anterior talofibular ligament injury

Case contributed by Lam Van Le
Diagnosis almost certain

Presentation

Swelling, pain, and limited ankle movement following an inversion injury of the left ankle.

Patient Data

Age: 35 years
Gender: Female

Increased thickness and signal of the anterior talofibular ligament (ATFL) on PDFS and T2-weighted images suggest a partial tear. The PTFL remains intact, with mildly increased signal intensity on PDFS and T2W sequences. Mild ankle joint effusion and soft tissue oedema in the lateral malleolar region are also noted.

Case Discussion

The imaging and clinical findings are consistent with a partial tear of the ATFL and anterolateral capsular injury.

The patient is being treated with conservative management, not surgery, using a compression bandage to limit ankle movement, ice application, NSAIDs for pain relief, and anti-oedema medication.
