The left basal ganglia and left anterior temporal cystic appearing lesions are most in keeping with perivascular spaces. Anterior temporal lobe perivascular spaces, only relatively recently described, have unique imaging appearances and frequently have surrounding edema that can wax and wane. Appearances, with the knuckle of MCA adjacent, are pathognomonic.
Previously described cystic appearing lesions of the basal ganglia and left anterior temporal lobe, are entirely stable in morphology. The left anterior temporal cystic lesion again lies adjacent to a branch of the left middle cerebral artery. No associated enhancement or increased cerebral blood volume (not shown). Spectroscopy traces are non-contributory (not shown). No abnormal diffusion restriction or susceptibility artefact (not shown). No other space-occupying lesion elsewhere.
Case Discussion
It is important to recognize anterior temporal pole perivascular cysts as benign lesions requiring no biopsy.
