Presentation

Incidental finding.

Patient Data

Age: 60 years

A well-circumscribed subcortical right anterior temporal cystic lesion is present with a halo of surrounding FLAIR signal abnormality. Small loculated components are seen at the anterolateral margin. A few prominent additional perivascular cysts are seen adjacent to the lesion. Additional CISS and TOF MRA imaging confirm vessel notching at the anterosuperior margin of the lesion, a classical appearance for perivascular spaces in this location.

Conclusion: Imaging features characteristic of an anterior temporal pole perivascular space.
