Aortic dissection with extension into aortic arch branches

Presentation

80-year-old female, presented with acute onset of dizziness, malaise and severe hypotension.

Patient Data

Age: 80 years
Gender: Female

There is dissection of the aorta involving the aortic root, ascending and descending aorta and abdominal aorta to just below the origin of the celiac axis (Stanford type A).

The dissection flap involves the root of the brachiocephalic trunk and left common carotid artery. There is also a dissection flap extending into the left subclavian artery to the level of the infra-clavicular vessel.

Case Discussion

This patient presented to ED with atypical symptoms. An urgent echocardiogram was arranged for her which revealed dilated aortic root and a probable dissected flap. The diagnosis of dissection was confirmed on CT.
